Applying for Retirement

 

Thinking of retiring? Learn what you need to do to get started…

The first step to retirement is to be prepared. On this page you will find a list of Retirement Application Package forms/documents you must submit to DERP to ensure a smooth transition.

A complete Retirement Application Package must be submitted to DERP within 30 days of your last day worked. Failure to submit a complete package will result in your retirement benefit payments being delayed and the loss of the Lump-Sum Death Benefit.

Insurance Enrollment/Change Form

Visit the Health Insurance webpage for more information and review the 
2020 Retiree Health Benefits Guide and determine which insurance plans you will enroll in. Choices include:

Certified Birth Certificate or Passport

DERP will not accept a birth registration, driver’s license, or hospital-issued birth certificate.

Social Security Estimate or Letter of Award

  • Social Security Estimate – Submit a copy of an estimate of your benefits from the Social Security Administration (SSA) if you are not currently receiving SSA benefits. Visit www.ssa.gov to obtain an estimate.
     
  • Letter of Award – Submit a copy of your original Letter of Award from SSA that lists your original entitlement date and gross amount of your SSA benefit if you are already receiving SSA benefits.

Spouse/Dependent Children Verification (if applicable)

  • Spouse – Submit a copy of your marriage certificate, common law affidavit, or the first page of your most recent federal tax return if enrolling your spouse on insurance.
     
  • Dependent children – Submit a certified birth certificate, guardianship paperwork, or adoption paperwork if enrolling your dependent children on insurance.

Consent of Spouse Form (if applicable)

Your spouse must complete this form and have his/her signature notarized if you elect the Maximum benefit or a Joint and Survivor benefit for a beneficiary other than your spouse.

Medicare Card or Medicare Entitlement Letter of Award (if applicable)

Ensure you have Parts A and B of Medicare in place on your effective date of retirement if eligible for Medicare on the date of your retirement (age 65 and older) and intend to enroll in a Medicare Advantage medical plan through DERP. To apply for Medicare Parts A and B, visit www.ssa.gov, or set up an appointment with a local Social Security office.
